Substance Use Disorder Counselor

Job Purpose

Clinical position responsible for providing individual and group counseling to patients and families in the outpatient substance use disorder counseling program as well as conducting partial hospitalization counseling. Duties and responsibilities include, but are not limited, to:

  • Maintain a patient caseload not to exceed 35 patients in the outpatient counseling program and a maximum of 10 patients in group PHP program.
  • Responsible for maintaining a case record on each individual patient addressing the components identified by Division of Licensing as required for reporting.
  • Responsible to provide updates and reports to referral sources on progress of referred patient.
  • Responsible to adhere to all confidentiality regulations governing patient treatment .
  • Attend training regularly and complete all required training forms .
  • Attend all staff meetings as scheduled and actively participate in initiatives designed to improve patient care
  • Insure that all records are up to date and accurate.
  • Schedule case consultations with Director of Substance Use Disorder Services to discuss ongoing patient progress.
  • Develop appropriate treatment plans for each patient assigned.
  • Participate with treatment planning, aftercare, discharge planning and aftercare efforts.
  • Responsible to assign patients assignments designed to enhance their recovery skills.
  • Provide patients with relapse prevention strategies.
  • Complete all information technology requirements for billing, reporting sources, etc.
  • Attend community meetings as assigned.
  • Provide educational programs on recovery and other aspects of drug and alcohol treatment

Qualifications

A Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 1 year of clinical experience (a minimum of 1,820 hours) in a health or human service agency, preferably in a drug and alcohol setting.
